Student Athlete Welcome Letter to help ensure that all of your information is submitted to the Athletic Training Department thoroughly and in a timely manner. Forms must be completed and returned no later than Monday, July 20, 2009 to:

Pre-Participation Physical Examination (PPE)
- All New Freshman And Transfer Student-Athletes
All New Freshman and Transfer Student-Athletes are required to complete a Pre-Participation Physical Examination with UNO Team Physicians prior to participating with Maverick Athletics. PPE’s performed by outside physicians will not be accepted. Athletes should arrive at the Sapp Fieldhouse dressed in
SHORTS AND A T-SHIRT and know the date of their last
Tetanus Vaccination.
Designated PPE Sport Dates are:
Wednesday, August 5 - 5:30 p.m. - Sapp Fieldhouse
Football (fall camp participants), Volleyball, Cross Country, Golf, Soccer, Dance, Cheer
Monday, August 24 - 6:00 p.m. - Sapp Fieldhouse
Men's Basketball, Women's Basketball, Track & Field, Hockey, Wrestling, Swimming & Diving, Softball, Baseball, Tennis, Football (all other participants)
Student Health Services Immunization Requirements
- All New Freshman And Transfer Student-Athletes
All newly enrolled UNO students are required to complete Student Health Services Immunization Forms. Failure to do so will result in loss of eligibility to register for classes making the student-athlete ineligible to participate in athletics. Your immunization status will be confirmed at the time of your physical.
